What Is Blepharoptosis?
Blepharoptosis, or ptosis, is the drooping of the upper eyelid that may affect one or both eyes.
When the muscles that control the upper eyelid fail, it can lead to a drooping appearance of the eye. The condition may be caused by a range of factors including genetics, aging, underlying medical conditions, and improperly administered cosmetic procedures such as botox.
Who Does Blepharoptosis Usually Affect?
Acquired blepharoptosis in adults is all too common. As we age, our skin loses its natural elasticity, which can lead to the gradual drooping of the eyelids.
Although blepharoptosis is more likely to affect older adults, it can affect children too. A child may be born with defective eye muscles which cause the eyelid to droop. A child suffering from blepharoptosis may tilt their head back in order to improve their vision.
Symptoms of Blepharoptosis
Symptoms of blepharoptosis include:
- The droopy appearance of one or both eyelids.
- Dry or irritated eyes.
- Impaired vision.
- Dull ache around the area of the eyes.
- Asymmetry of the eyes.
Acquired vs. Congenital Blepharoptosis
Acquired blepharoptosis is the drooping of one or both eyelids that develops over time, most likely due to aging. On the other hand, congenital blepharoptosis refers to the drooping of the eyelid which has been present in the individual since birth.
Congenital blepharoptosis is often only treated by invasive treatments such as surgery. However, acquired blepharoptosis may be effectively treated through the use of Upneeq eye drops. Treatment options for acquired blepharoptosis vary, however, most patients with acquired ptosis opt for the Upneeq eye drops due to the non-invasive nature of the treatment.
Can Botox Cause Blepharoptosis?
Botox is normally used to smoothen wrinkles, crow’s feet, and signs of old age that are primarily visible on the face. The formula in botox prevents the muscle from moving in order to achieve the desired youthful cosmetic effect.
When botox is administered incorrectly, it can cause multiple issues. Botox is often injected into the forehead to improve the appearance of wrinkles. However, when botox is injected into the forehead incorrectly, the substance can inadvertently move towards the eyelid muscles.
In turn, the muscles that control the eyelids’ movement become weakened, and thus the eyebrow can slightly drop. As the eyebrow lowers, it can push on the eyelid, creating the illusion of a droopy eyelid.
Fortunately, the effects of botox tend to subside after 3 to 4 months. Nevertheless, while patients wait for the botox to naturally wear off, the effects of the misaligned substance administered close to the eye can cause great insecurity if it causes the eyelid to droop significantly. Upneeq can be used as an effective treatment to lessen the appearance of a low-lying lid inadvertently caused by botox.
What Is Upneeq (Oxymetazoline Hydrochloride Ophthalmic Solution 0.1%)?
Upneeq oxymetazoline hydrochloride ophthalmic solution 0.1% is an eye drop designed to reduce the signs and symptoms of droopy eyelids.
When dropped into the eye, oxymetazoline is an alpha-adrenergic agonist that stimulates the upper eye muscles known as the Müller’s muscle. Alpha-adrenergic agonists cause the Müller’s muscles to contract, thus raising the eyelid.
How Effective Is Upneeq?
Upneeq has proven to be an effective treatment in the lifting of the eyelids. In fact, on average, studies show that patients who were treated with the eye drop saw a 1mm lift in the eyelid.
Studies show that most patients experienced a visible improvement in their drooping eyelid(s) and most results were apparent within just 15 minutes of administering the eyedrop. In fact, 84% of patients noticed a visible improvement in their droopy eyelids.
It is important to note that this treatment may not be so effective in the improvement of severe blepharoptosis that may impair vision. However, Upneeq may be used as a cosmetic treatment to noticeably improve the appearance of low-lying lids.
Upneeq vs. Eyelid Lifting Surgery
Upneeq can be used as a temporary treatment of acquired blepharoptosis. The effects of the eyedrop can last for up to 6 hours to give the low-lying lids a well-needed lift. However, effects are only really visible in mild cases of blepharoptosis.
In other words, patients experiencing more severe cases of blepharoptosis would benefit more from eyelid lifting surgery. Patients with mild blepharoptosis who are seeking an alternative treatment to invasive surgery can rely on Upneeq eye drops to improve the appearance of droopy eyelids.
Side Effects of Upneeq
Some adverse reactions that have been recorded in patients taking Upneeq were punctate keratitis, conjunctival hyperemia, dry eye, blurred vision, eye irritation, and headache. We advise patients to seek medical care if signs of adverse reactions are apparent when using Upneeq eye drops.
If the patient experiences severe eye pain, dizziness, or impaired vision after administering Upneeq, they should seek immediate medical care.

Warnings and Precautions
Patients receiving alpha-adrenergic receptor antagonists present in Upneeq must be aware of the complications that may occur when using them together with other prescribed medicines. Patients with certain medical conditions should also practice caution before using Upneeq eyedrops.
- High blood pressure – Alpha-adrenergic agonists, as a class, may impact blood pressure, which is why we advise patients with cardiovascular disease, orthostatic hypotension, and hypotension to seek medical advice before using Upneeq.
- Angle-closure glaucoma – patients with untreated narrow-angle glaucoma or that show symptoms of acute narrow glaucoma should not use Upneeq as it may increase the risk of irritation and discomfort in the eyes. The risk of angle-closure glaucoma symptoms in patients using Upneeq may increase.
- Monoamine oxidase inhibitors – care must be taken by patients taking monoamine oxidase inhibitors who want to use Upneeq eyedrops. Users may experience a difference in their metabolism and a decrease in libido when using their prescribed medicine together with the Upneeq eye drops.
Is Upneeq Right for Me?
If you believe one or both eyelids have drooped over time, then Upneeq may well be an effective treatment for you.
If the patient suffers from conditions such as an aneurysm, Horner syndrome, myasthenia gravis, or acute narrow-angle glaucoma, then other treatments including surgery may be a preferred course of action when dealing with said medical issues.
Patients who suffer from immune disorders such as Sjögren’s syndrome which leads to a drying of the mouth and/or eyes should practice caution and speak to a medical professional before using Upneeq eye drops.
Is Upneeq Safe to Use?
When deciding whether or not to use a new product, it is extremely important to determine how safe it is to use. Therefore, it is good to know that Upneeq is completely FDA approved.
During its first 6-week trial which included over 300 patients, Upneeq was found to be both a safe and effective treatment of blepharoptosis. Two additional studies later confirmed this result.
Administration and Dosage
Upneeq should be administered once daily in one or both of the affected eyes.
Patients must wait 15 minutes after administering Upneeq before using contact lenses or other eye medicines including eye drops.
When administering Upneeq, be sure to avoid contact between the container and the eye in order to prevent eye injury or contamination. We advise consulting a medical professional before using more than one eyedrop or other eye care medication.
