Program Rules
- All plans have a $25 set-up fee.
- 12-month commitment required with EFT auto draft.
- Subscription benefits cannot be combined with any other offers or promotions.
- Botox units and other subscription benefits are non-transferable.
- Unused Botox units expire at the end of 12 months.
- Units can only be used once they are accrued.
- Special pricing is only available to current subscribers and is non-transferable.

Terms & Conditions
Cancellation Fee: The standard $50 cancellation fee does not apply once twelve months of consecutive membership is achieved.
Memberships will be in place for a minimum of twelve months from the signup date. Cancellations before that twelve-month period will be subject to a cancellation fee equal to the price difference of the member price versus the regular price of the services received plus a $50 cancellation fee.
Appointment no-shows and same-day cancellations will be deducted for the full service and tardiness will be deducted from the appointment time when necessary. Please arrive 10 minutes before your scheduled appointment time.
Membership benefits carry no cash value and are non-refundable. Memberships are non-transferable but day-spa meberships may be shared with individuals who are added to your shared list.

Accrued benefits expire immediately upon the cancellation of membership. Memberships may be frozen for no more than a total of three months per twelve-month period.
Memberships that go unpaid will be canceled 10 days after the billing cycle (the 1st of the month) and all accrued benefits will be lost. Please notify us of changes in your card number, expiration date, etc. to avoid unnecessary closure of your membership.
